WELCOME and Thank You for choosing Missouri Slope Lutheran Care Center. We want to make your transition as easy as possible.
You will need to
complete: 1) Application for admission, 2) Pre-entrance medical record and 3) Admission transfer orders. On the day of admission please bring the
following:
- Social Security Card
- Medicare Card
- Additional Insurance Cards: Name and Policy Number
(For Nursing Home Insurance bring the policy)
- Medicaid Number
- Name, address and telephone numbers of any family or
friends that you want listed as primary contact people
- Copy of Advance Directives, such as:
Durable Power of Attorney
Durable Power of Attorney for Health Care
Power of Attorney
Guardianship
Living Will
- Bring Bottles of Medications from home.
- Clothing:
Coat
Housecoat
Slippers
Couple pairs of shoes
5-7 sets of clothes
- Personal Items (optional):
Recliner
T.V.
Pictures
Bedding
etc.
(All items on the day of admission will be inventoried and marked. If bringing in items later please take to the nurse's station.)
